To begin our Potential Client Intake process, please fill out your answers to all of the below questions as best as possible.

Within 7 business days of your submission, we will email you a confidential Dropbox link so that you can send us all of the following so we can get started in evaluating your Potential Case:

• • 1. 5-15 photos and videos of your worst uninhabitable living conditions in the past two years;

• • 2. 5-15 communications with your Landlord/Management about the issues/requests for repairs in the past two years (including your most recent communication);

• • 3. A copy of your lease or purchase agreement
